I do hereby release The Children’s Playhouse and its staff from any and all liability for injury, illness, or any other condition of my child(ren) that may occur while at The Children’s Playhouse, during participation in any activities or programs of The Children’s Playhouse, or as a direct or indirect result of participation in any activities or programs of The Children’s Playhouse, and waive my right to pursue legal action against The Children’s Playhouse or its staff or volunteers in connection with any such, injury, illness or condition. I understand that --my child(ren) must be accompanied by an adult at all times while at The Children’s Playhouse. --The Children’s Playhouse provides resources and activities designed for children from birth through eight years of age and that there may be activities that are not age appropriate or developmentally appropriate for my child(ren)’s participation. In addition, some activities may not be appropriate for my child(ren) because of a physical, medical, or other condition of my child(ren). --As the parent or caregiver, I am responsible for monitoring the activities of my child(ren) while at The Children’s Playhouse and choosing activities that are appropriate for my child(ren). --The Children’s Playhouse also reserves the right to limit my child(ren)’s participation in an activity based on safety issues or other program-related concerns. --the primary goal and responsibility of The Children’s Playhouse staff is to facilitate participation in creative learning activities and not to serve as caretakers to my child(ren). Further, in recognition of the COVID-19 pandemic, I also understand that: --I and my child(ren) will not be granted access to The Children’s Playhouse if either I, my child(ren), or any members of my immediate household have symptoms of COVID-19, are suspected to have COVID-19, or have been recently diagnosed/confirmed with COVID-19. --The Children’s Playhouse has taken steps to implement recommended guidance and protocols issued by the Centers for Disease Control (CDC) and the American Academy of Pediatrics for slowing the transmission of COVID-19, including, without limitation, requiring frequent hand washing by all staff and attendees, sanitation of items used in The Children’s Playhouse, requiring masks for ALL staff and attendees, as well as the other access/use restrictions set forth above. I acknowledge and agree that The Children’s Playhouse may revise its procedures at any time based on updated recommended guidance and protocols issued by the CDC and further agree to comply with The Children’s Playhouse procedures when at The Children’s Playhouse. --I fully understand and appreciate both known and potential dangers of attending in-person public events and acknowledge that attendance by my child(ren) and/or myself may, despite, the reasonable efforts of The Children’s Playhouse to mitigate such dangers, result in exposure to COVID-19, which could result in quarantine requirements, serious illness, disability, and/or death.

Photography and video recordings from The Children’s Playhouse help us convey the value and excitement of the Playhouse experience. We use photos to show funders and to enhance the quality of our print materials, newsletters, the Playhouse website, social media, and publicity and general fundraising. Your picture will not be used in conjunction with scholarship fundraising without your additional explicit, written consent. I, the parent or guardian of the children included in this membership, the undersigned, grant permission to the The Children’s Playhouse to photograph, tape, film, or make an audio recording of my/our child\'s participation at The Children’s Playhouse or in Playhouse sponsored events. I/We grant the The Children’s Playhouse an irrevocable license to reproduce, copy, display, perform, or otherwise use these materials. I/We understand that these materials will be used for the purpose of public information and/or education and/or marketing of The Children’s Playhouse and may appear in newspapers, magazines, exhibits, The Children’s Playhouse publications, on The Children’s Playhouse website, social media, or on television. I/We agree that my/our child may be identified as a participant in the event being recorded. I/We release the The Children’s Playhouse, its employees, directors, and agents from any liability connected with the publication, reproduction, release, or other use of these materials, and agree not to bring any claims against them growing out of such publication release, reproduction, or other use of these materials.   ____ I agree to the photo release ___ I do NOT agree to the photo release  If I selected \"do not agree,\" I understand that I must request and wear a “no pictures” sticker when visiting The Playhouse and/or I will verbally inform anyone taking pictures to not take our pictures.
