I do hereby release The Children’s Playhouse and its staff from any and all liability for injury, illness, or any other condition of my child(ren) that may occur while at The Children’s Playhouse, during participation in any activities or programs of The Children’s Playhouse, or as a direct or indirect result of participation in any activities or programs of The Children’s Playhouse, and waive my right to pursue legal action against The Children’s Playhouse or its staff or volunteers in connection with any such, injury, illness or condition.
I understand that my child(ren) must be accompanied by an adult at all times while at The Children’s Playhouse
the primary goal and responsibility of The Children’s Playhouse staff is to facilitate participation in creative learning activities and not to serve as caretakers to my child(ren).
The Children’s Playhouse provides resources and activities designed for children birth through eight years of age and that there may be activities that are not age appropriate or developmentally appropriate for my child(ren)’s participation. In addition, some activities may not be appropriate for my child(ren) because of a physical, medical, or other condition of my child.
As a parent, I am responsible for monitoring the activities of my child(ren) while at The Children’s Playhouse and choosing activities that are appropriate for my child(ren).
The Children’s Playhouse also reserves the right to limit my child(ren)’s participation in an activity based on safety issues or other program-related concerns.

I understand that pictures are sometimes taken at The Children\\\'s Playhouse for educational or publicity purposes. If I do not want my or my family\\\'s pictures taken, I will tell staff and complete a no-photos form and ask for a \\\"no photos\\\" sticker to wear if desired. If the No Photos form is not completed, you are affirming the following release: Photo Release I/We grant permission to the The Children’s Playhouse to photograph, tape, film, or make an audio recording of my/our child\\\'s participation at The Children’s Playhouse or in Playhouse sponsored events. I/We grant the The Children’s Playhouse an irrevocable license to reproduce, copy, display, perform, or otherwise use these materials. I/We understand that these materials will be used for the purpose of public information and/or education and/or marketing of The Children’s Playhouse and may appear in newspapers, magazines, exhibits, The Children’s Playhouse publications, on The Children’s Playhouse website, social media, or on television. I/We agree that my/our child may be identified as a participant in the event being recorded. I/We release the The Children’s Playhouse, its employees, directors, and agents from any liability connected with the publication, reproduction, release, or other use of these materials, and agree not to bring any claims against them growing out of such publication release, reproduction, or other use of these materials. I understand that no compensation is offered for the use of these images.
